How to Submit an Information Request Form

Information Request Submission Process

The Data Governance Group has created a process to request data, application access or report access via a forms submission process. These requests are managed within the ITS service desk software (Team Dynamix) with all your other requests. 

The purpose of the request forms is to help us better serve you and to allow us the opportunity to track and prioritize requests. Your request will be reviewed and assigned to the appropriate responsible office. We intend to meet these requests in a timely manner.

All requests will adhere to the guidelines set by the legal entities and organizations that apply to the information maintained by Wartburg College.

How to submit the requests:

  1. Navigate to https://www.wartburg.edu/techline
  2. Ensure you are logged in by clicking on Sign in the upper right corner

  1. Select the appropriate service request and complete the form as shown.

  1. Application Access Form
    • Click the button that says Request Access

  • Fill out the form as needed; help is displayed by each field for your assistance.

  • Click the Submit button

    • Click the button that says Request Access

  • Fill out the form as needed; help is displayed by each field for your assistance.

  • Click the Submit button

    • Click the button that says Request Data

  • Fill out the form as needed; help is displayed by each field for your assistance. This is the form where you would enter a user story. Please enter as best you can using simple terms.

  • Click the Submit button

Once your request is submitted, you will be able to manage the request in the service desk software as you would any other ITS request. You will also receive an email from the ticketing system and be able to respond to your ticket via email or the links included in the message.
